Years of being a massive nerd, huddled over keyboards and mobiles has left me with terrible "nerd neck" and rounded shoulders. It's not as bad as some people who I work with (who seem to have a 90deg neck, somehow) but it's still not the best.
I did "fix" this once, when I was in my gym phase during my 20's, my PT gave me various exercises to help, but over the years I've literally slumped back into it. It's a painful, difficult thing to break out of - pull-up's seem to help, as do push ups.
If you've got kids, it's a horrible habit for them to have with phone/tablet use.
Also, fun fact - good posture has a proven* link to career success. Unless you work in IT...then it's probably the opposite.

I do Pilates which helps my general posture, all about the core strength init.

Another big fan of Pilates here. Been doing it for a couple of years, and I really wish I'd discovered it ten years earlier, and saved myself a shit load of money with physios, osteopaths, and charlatan chiropractors.

My top tip is sorting your core out and your upper back wi follow. Squats, kettlebell lunges and other things directed towards addressing an anterior pelvic tilt.
