I worked for a Citroen/Seat dealer in the mid 90s and got to drive a lot of these that had been taken in part exchange. Back then Seats were shit in every respect so they must have been trader in to Citroens. Looks more like a cam snapping or releasing from the crack to me. There’s a hell if a lot of fake climbing gear available cheaply on the internet that looks the part until something like this happens
These things also have a lifespan and a load limit, like if you fall on one of these cams some say you should dispose of them and not reuse anymore

My climber mate spends an inordinate amount of money on climbing headwear, rotating stuff out very regularly.
